The only real evidence Social Security has of your disability, other than your testimony, is your medical records. This is why it is critical to see your doctor regularly and ensure your doctor records and documents your disability symptoms and disability impairments.

Importance of Documenting Symptoms and Impairments

What do we mean by ‘document’ your disability symptoms and impairments? Make sure you are telling your doctor all of your symptoms and impairments every time you go in and make just as sure that your doctor is writing them down. Having well documented medical records and doctor’s notes is a critical component for presenting your Social Security disability claim.

Tell your doctor you can’t sit more than 15 minutes without significant pain as opposed to just saying you have trouble sitting. Be specific. Tell your doctor you can’t walk more than a block as opposed to just saying it hurts when you walk. If you have to lie down several times per day, tell your doctor how often and for how long.
